MATCH APPLICATION

This is a preliminary match registration form. You may need to complete and sign the official form at the match. Please fill this out and mail it to the address as listed in the match bulletin along with registration fees in sufficient time to allow Squadding.

If this registration is for a CMP "Sanctioned" match such as an EIC or Garand Match. You will need to have an "AFFIDAVIT of ELIGIBILITY" form notarized and on file prior to the match. This form can be downloaded from the High Power page of the www.usrpa.org web site and mailed in with this application.

If this match is for an NRA Approved, State or Regional Match, you must be a current member of the National Rifle Assoc and the Utah State Rifle and Pistol Assoc, or a member of your local State Assoc. Proof Required, Applications will be available at range.

DEADLINE: This form should be recieved no later than 5 pm, 3 days prior to the match to facilitate advanced squadding. Applications will be accepted at the range 30 minutes before the scheduled start of the match.

The match Bulletin will be posted at the Utah State Rifle and Pistol Associations web page http://www.usrpa.org/High_Power/high_power.html. If you do not have access to the internet, we will mail the bulletin.
